    Please let me know if you have insurance you'd like to use to pay for your therapy.  I can check on your benefits if you give me your insurance ID# and your date of birth.

    Let me know your availability for appointments (like if you are available only on certain days or times),  a few sentences about the issues you'd like to work on, and why you'd like to work with me.

    Please be aware that I do not work after 5 pm or on weekends.
